Topic:  Navigating the Spectrum of End-of-Life Care: A Clinical Guide for Healthcare Social Workers.  Presented by: Cara Donaldson, LCSW-BACS, CCM, MBA.  During this presentation participants will:  1. Differentiate between Palliative Care, Hospice and Death Doulas regarding definition, history, eligibility, and philosophy.  2. Synthesize and apply clinical framework (Family Systems, Attachment, and Crisis Intervention theories) to end-f-life care coordination and therapy.  3. Diagnose and clinically address end-of-life distress and anxiety utilizing the DSM-5 framework.  4. Evaluate the physiological stages of the active dying process to provide anticipatory guidance to families.  5.  Identify insurance coverage boundaries, including Medicare, Medicaid, and private insurance parameters.  6. Effectively connect clients and supplementary support services, such as sitters, adult day health care, and respite options.  2.00 Clinical CEU hours.  Pre-registration is required to be awarded CEU credit.
